Referring Providers

If you would like to refer a patient to Wise Hearing Care PLLC, please send a fax to (224) 350-0120. Be sure to provide the patient’s contact information and insurance details. Wise Hearing Care accepts Medicare and many private insurance plans, but we do not accept Medicaid at this time.

You can expect a thorough and accurate audiologic evaluation, with a comprehensive report sent to your office within 24 hours of the appointment. Wise Hearing Care sees individuals 7 years and older.

Services Not Offered:

• Evoked potential testing (e.g., ABR)
• Vestibular testing
• Cochlear implant candidacy evaluations
